The Community Re-entry Program offers two ways for resident to live in the community: supported apartment living and monitored community living. Based in a public apartment complex in Austin, Texas, the program provides a least restrictive treatment alternative for individuals who are ready for semi-independent community living. Placements also include individuals who are in transitional training to return home. The programs are tailored to each resident's needs and supervised by professional support staff. The length of stay can vary from six months to life-long care.
Residents in the Community Re-Entry Program must maintain a job or be enrolled in vocational training programs or educational activities, for at least 20 hours a week. All residents are required to participate in therapeutic group activities and meet their personal goals for independent living.
Residents accepted into the program must be at least 18 years of age and be medically stable. Individuals with behavioral problems are not yet ready for living in a semi-independent environment.
This program offers residents a chance to live more independently in the community while continuing to receive just the right amount of support services to prevent a stay at a more restrictive setting. Although this is an unlicensed program, CORE Health Care's full range of licensed therapies, behavioral management, and neurological and medical reviews. A CORE Health Care staff member is available to residents 24 hours a day.
In the apartment program, residents reside in one public complex, with direct care and professional staff available at a central staff office. Since the residents have independent daily schedules and are not visually monitored twenty-four hours a day, candidates must be able to initiate consistency with medication compliance and regular accountability. Resident must also demonstrate personal accountability: answering their phone and door consistently and notifying program staff of their arrival and departure. Residents are encouraged to have family and friends over to their apartment.
In the monitored community living program, residents maintain their own apartments and houses in and around Austin. Our professional staff remains "on-call" for a small fee and we can visit the resident on an as needed basis for recreational activities, shopping, medical appoints or any other service. Residents of the monitored community living program are free to join the apartment program for social and therapeutic groups.
