Post Acute Brain Injury Rehabilitation Program

 

CORE Health Care provides a homelike setting for brain injury rehabilitation.The Hilltop Treatment Program, a small residential facility, meets the needs of individuals who have suffered mild-to-moderate brain injuries. CORE Health Care's goal is to restore self-directed functional ability to individuals with a brain injury so that they can live in the least restrictive environment possible with minimal need for support services. Since biological recovery of an injured brain continues for two years or more post-accident, the typical length of stay at Hilltop for someone with a brain injury ranges from six to eighteen months. Program participants must be medically stable and age 18 or older.

Specific emphasis is placed on improving each resident's self-awareness, ability to set personal goals, interpersonal skills, emotional amd behavioral control, and adequate judgement and problem solving.Rehabilitation for brain injuries.CORE Health Care provides a homelike setting for brain injury rehabilitation. CORE Health Care's treatment strategies are specific to the resident's needs for improved physical, cognitive, behavioral and psychological functioning. Family issues are also addressed by the resident's treatment plan. In a beautiful ranch-like environment, residents receive physical, occupational, and speech therapies, including such unique activities as therapy with music, art, gardening or animals. CORE Health Care's licensed professional staff also provides cognitive testing and therapeutic intervention plans for those with brain injuries. Direct care personnel foster a consistent but challenging therapeutic environment for residents learning a variety of compensatory strategies. CORE Health Care's staff members are experts in brain injury rehabilitation; many have been certified through the Brain Injury Association of America as Brain Injury Specialists.

Hilltop residents experience qualitative functional outcomes through a consistent and predictable schedule, peer support, individual and group therapy, multi-level intervention and an optimistic environment. Each resident has their own private bedroom and half bath.
